How We Use and Disclose Your Health Information

We may use and disclose your health information for the following purposes:

  • Treatment: We may use your health information to provide, coordinate, or manage your healthcare. This includes sharing information with doctors, nurses, specialists, hospitals, and other healthcare providers involved in your treatment.
  • Payment: We may use and disclose your health information to obtain payment for services provided. This includes billing insurance companies, Medicare, Medicaid, or third-party payers.Healthcare

Under the Health Insurance Portability and Accountability Act (HIPAA), you have the following rights regarding your health information:

Right to Access Your Records

You have the right to inspect and obtain a copy of your medical records and other health information that we maintain.

Requests must be made in writing, and we may charge a reasonable fee for copying and mailing.

Right to Request Amendments

If you believe the information in your records is incorrect or incomplete, you may request an amendment in writing.

We may deny your request if we determine that the existing information is accurate and complete.

Right to Request Confidential Communications

You may request that we communicate with you about medical matters in a specific way or at a certain location, such as only by mail or phone at work.

We will accommodate reasonable requests.

Right to Request Restrictions on Disclosures

You may request restrictions on how we use or disclose your health information for treatment, payment, or healthcare operations.

We are not required to agree to your request, except in cases involving out-of-pocket payments where you request that we do not disclose information to your health plan.

Right to an Accounting of Disclosures

You have the right to receive a list (accounting) of certain disclosures of your health information made by us, excluding disclosures for treatment, payment, or healthcare operations.

The request must be in writing and may cover up to six years of disclosures.

Right to a Paper Copy of This Notice

You may request a paper copy of this notice at any time, even if you have agreed to receive it electronically.

Right to File a Complaint

If you believe your privacy rights have been violated, you may file a complaint with our office or with the U.S. Department of Health and Human Services (HHS), Office for Civil Rights.

Filing a complaint will not affect your care or treatment.
